29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems:      a.  On or about May 15, 2019, at the above addressed jobsite, employees performing residential roofing loading operations on a roof, nine (9) feet ten (10) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

On or about May 15, 2019, at the above addressed jobsite, an employee was carrying equipment (portable leaf blower) up a portable ladder used to access a roof, thereby exposing the employee to the hazards associated with falls.

On or about May 15, 2019, at the above addressed jobsite, employees had been provided fall protection training, however the employer failed to keep a written certification of record.
