29 CFR 1910.22(d)(1): The employer did not ensure that Walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition; (a) Frontier North, Inc. - On or about May 21, 2019 employees were exposed to slip, trip and fall hazards due to walking-working surfaces not being inspected regularly allowing conditions to exist, such as but not limited to: the storm grate cover frame was corroded, cross bars broken, bent and in deteriorating condition, missing portions of the storm cover supporting frame making the cover unstable.
