JOBSITE:  The employees performing tasks, such as (but not limited to) connecting a water main to a valve in the 5 feet 2 inches to 5 feet 7 inches deep, 5 feet wide and 32 feet long trench were not protected from the hazard of cave-ins by the use of appropriate cave-in protection systems/devices.    On or about 05/17/2019, the north wall of the trench caved in causing one of employees performing the above task to sustain a fractured pelvis.
