Tiger, wet side - Employees were exposed to a fall hazard while working from an A-frame ladder (i.e. Louisville, Model FM1504, 4' length) having a locking device and top step bent. b. Tiger, dry side - Employees were exposed to fall hazard of 17' to floor level while accessing an area using a fixed ladder having a siderail not securely bolted to the floor. c. Conveyors area - Employees were exposed to fall hazard of 12' to floor level while accessing an area using a fixed ladder having a which was bent and unbolted to the floor.
