29 CFR 1926.403(i)(2): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures:  a) 120 Second Street, Basement, Worksite - On or about May 20, 2019, employees were working in the area of the main electrical control panel, with exposed live parts energized in excess of 50 vAC, where front cover and partial dead front had been removed.  b) 120 Second Street, Second Floor, Worksite - On or about May 20, 2019, an employee was working in the area while utilizing a 110 vAC receptacle that had its proper cover removed.    c) 120 Second Street, Second Floor, Worksite - On or about May 20, 2019, an employee was working in the area of a 110 vAC splice of electrical conductors that were not located in an enclosed electrical box.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) 120 Second Street, Worksite - On or about May 20, 2019, employees were performing demolition work on the second floor of a residence, approximately 18 feet above grade, without utilizing any means of fall protection.

29 CFR 1926.850(c): All electric, gas, water, steam, sewer, and other service lines were not shut off, capped, or otherwise controlled, outside the building line before demolition work was started. In each case, any utility company which was involved was not notified in advance.  a) 120 Second Street, Worksite - On or about May 20, 2019, employees were conducting demolition operations but the employer had not notified the electrical utility that the work was underway nor ensure the service was shut off prior to exposing the live parts of the electrical circuits.
