29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) On or about May 13, 2019, the employer did not ensure that employees conducing tile roofing operations on a new single-family residential home at 6777 Dunkirk Ct., Centennial, CO 80016 were protected from fall hazards greater than 10 feet. 29 CFR 1926.1153(c)(1): Specified exposure control methods. For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section:    (a) On or about May 13, 2019, employees were using hand held circular saws to cut concrete roofing tiles that were not equipped with integrated water delivery systems or other adequate dust suppression systems per the engineering and work practice controls specified in Table 1.    Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1153(d)(2)(i): The employer did not assess the exposure of each employee who was or may reasonably be expected to be exposed to respirable crystalline silica at or above the action level in accordance with either the performance option in paragraph (d)(2)(ii) or the scheduled monitoring option in paragraph (d)(2)(iii) of this section:    (a) On or about May 13, 2019, employees were utilizing Rigid brand hand-held circular saws to cut concrete roof tiles and the employer did not conduct an exposure assessment of employees exposed to respirable crystalline silica during this process.
