29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  On or about May 22, 2019, during roofing activities at a job site located at 2047 16th Street, Cuyahoga Falls, Ohio 44223; the employer did not ensure employees were protected from electrical shock hazards by ensuring the electrical power circuits of the service drop line were de-energized or guarded. The toe board was in close proximity to the service drop line. There was a tarp draped over the front of the house that was touching the energized service drop line.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 23, 2019, during the installation of residential roofing, the employer failed to ensure that an aluminum ladder being used to access the roof, was extended at least 3 feet above the landing surface. The approximate height of the landing access point was approximately 19 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about May 23, 2019, during the installation of roofing material at a job site located at 2047 16th Street, Cuyahoga Falls, Ohio 44223; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 21 feet by utilizing a fall protection system.  Franklin Javier Miranda Damas, was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1356254 citation number 1, item number 3 and was affirmed as a final order on 1/7/2019, with respect to a workplace located at 4422 3rd Street Northwest, Canton, Ohio 44708.
