29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired: a) On or about May 22, 2019, the Werner 6 Foot Rolling Scaffold, model SRS-72, had the wood decking damaged in that it had wide splinter sides and a crack below coming from carrying holes, exposing employees to fall hazards.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards: (a) On or about May 22, 2019, for employees using a Werner 6 Foot Rolling Scaffold, model SRS-72, with damaged wood decking material that had splintering sides and a crack coming from carrying holes.
