29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a) On or about 5/21/19 at a tree removal project located at 2607 Ridge Road, Mount Morris, New York; employees were using a crane while removing a tree and placing the logs roadside for later pick up by others. The crane contacted 7200 volt power distribution line running along the road while an employee was holding onto the rigging chains. The employee was electrocuted. The employer did not ensure employees were trained in identifying, recognizing, or the hazards associated working in proximity to power lines nor were they trained in methods to avoid contact with power lines. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: a) On or about 5/21/19 at a tree removal project located at 2607 Ridge Road, Mount Morris, New York; employees were using a crane while removing a tree and placing the logs roadside. The employer did not ensure the nearby overhead lines were deenergized and grounded or protected from contact by any parts of the crane. The hoist cable made contact with the energized line electrocuting an employee handling rigging attached to the hoist cable. ABATEMENT DOCUMENTATION REQUIRED

The crane operator moved the crane boom near an unguarded, energized overhead line. The hoist cable on the crane made contact with the energized line electrocuting an employee on the ground that was handling the rigging attached to the hoist cable. ABATEMENT DOCUMENTATION REQUIRED
