On or about May 22, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a.  On or about May 22, 2019, at the above addressed jobsite, employees were exposed to falls while performing stucco and mason screening operations from a tubular welded frame scaffold,  four (4) tiers and one (1) tier above the ground, that did not have the working level fully planked.

On or about May 22, 2019, at the above addressed jobsite, an employee performing stucco operations from a tubular welded frame scaffold with a height to base width ratio of more than four to one (4:1) and was not secured with guys, ties and/or braces, thereby exposing employee to fall hazards.

Crossbraces were used as a means of access.   a.
