29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.   a) On or about May 8, 2019; East-side (front) area; the scaffold system erected had several potential fall hazards which the competent person should have recognized and abated during the installation process such as but not limited to fall protection, missing toe-board and lack of railing .    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).  a) On or about May 8, 2019; east-side (front-area); the employees were exposed to an estimated 20-foot fall while working near the unguarded ends of the scaffold system while conducting masonry work.    No abatement certification or documentation required for this item.

No abatement certification or documentation required for this item.
