29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:    On or about May 22, 2019, employees were exposed to amputation hazards when performing maintenance and servicing on equipment, including but not limited to, the Amada Coma Machine, and the employer did not developed machine specific procedures.     The procedures shall clearly outline: A specific statement of the intended use of the procedure, Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy, Specific procedural steps for the placement, removal and transfer of lockout devices and the responsibility for them, and  Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

The certification shall include identification of the machine or equipment, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:     On or about May 22, 2019, employees were exposed to hazardous energy sources when performing maintenance and servicing, including but not limited to the Waldemar Initial Pinch Bending Roll Machine, and the employer did not provide training.     The training shall include the following:        a. Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.        b. Each affected employees shall be instructed in the purpose and use of the energy control procedures.        c. All other employees who work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about May 22, 2019, employees were exposed to hazardous energy sources when performing maintenance and servicing, including but not limited to the Amana Coma Machine, because the employer did not did not cover the action listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6).    Employees who performed servicing and maintenance were not required to:       (1)   Know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,       (2)   Turn off and shut down the machine,       (3)   Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,       (4)   Affix lockout or tagout devices to each energy isolating device,       (5)   Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.       (6)   Prior to starting work on machines that have been locked out or tagged out, verify that isolation and de-energization of the machine or equipment have been accomplished.
