This condition exposed employees to flying chips and particles.

This condition exposed employees to fall distances of 10 feet and 18 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) D Hernandez Construction, Inc. at 618 W 175th Ave. Broomfield, CO 80023: On or before 5/23/19 the creating and exposing employer did not protect employees from injury in that employees working on the roof of a new two story single family home were accessing the first and second levels with ladder which did not extend at least three feet beyond the eaves of the roof. This condition exposed employees to fall hazards.
