29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On or about May 23, 2019, in the front of a commercial building under renovation, two employees were performing stucco work on a one tier tubular welded frame scaffold without the platform being fully planked, exposing the employees to a fall hazard of approximately 9 feet and 6 inches.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about May 23, 2019, in the front of a commercial building under renovation, two employees were performing stucco work on a one tier tubular welded frame scaffold without the employer providing scaffold training, exposing the employees to a fall hazard of approximately 9 feet and 6 inches.

29 CFR 1926.451(e)(7): Steps and rungs of ladder and stairway type access did not line up vertically with each other between rest platforms:  On or about May 23, 2019, in the front of a commercial building under renovation, two employees accessed the scaffold platform located approximately 9 feet and 6 inches above ground utilizing integral prefabricated scaffold access frames that did not line up vertically.
