No materials or equipment may be placed, either permanently or temporarily, within the exit route:  (a)    Oak Creek Rehabilitation Center of Kimberly, South Side Evacuation Route:  On or about May 21, 2019 and times prior:  Items including but not limited to an aluminum ladder, table, wet vacuum, floor buffer and shelving were stored in the south side evacuation route. This exposes employees to possible delayed egress hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  Oak Creek Rehabilitation Center of Kimberly, Medical Records / MDC Coordinators Office:  On or about May 21, 2019 and times prior:  A Frigidaire refrigerator was plugged into a surge protector (relocatable power tap).  The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  (a)    Oak Creek Rehabilitation Center of Kimberly, Kitchen:  On or about May 21, 2019 and times prior:  The True brand stainless steel refrigerator in the kitchen had the outer sheath of the wiring pulled from its plug casing and cord clamp.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a)  Oak Creek Rehabilitation Center of Kimberly, Maintenance Office:  On or about May 21, 2019 and times prior:  Master Flame fire retardant liquid was in an unlabeled spray bottle (secondary container).    (b)  Oak Creek Rehabilitation Center of Kimberly, Maintenance Office:  On or about May 21, 2019 and times prior:  A Marigold butter container (secondary container) contained latex paint.
