29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about May 22, 2019 the employer failed to ensure that an annual inspection of the energy control procedures for the equipment at the facility had been performed. Employees are tasked with servicing and maintaining equipment such as but not limited to shears, saws, and slitters. Hazardous energies present on equipment includes but is not limited to pneumatic, mechanical, electrical, and hydraulic.

Employees operate equipment such as but not limited to slitters and shears.

29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.    On or about May 22, 2019 the employer failed to ensure that the "big blue" slitter was LO/TO in accordance with the procedures in place. A maintenance employee was servicing the equipment without the use of locks on the cutoff switches of the main control panel. The machine or equipment was not turned off or shut down as required.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks;    a) On or about May 22, 2019 the employer failed to ensure that effective machine guarding was in place on the #2 shear within the facility. The back portion of the shear was not guarded to protect employees from reaching the hazardous area.     b) On or about May 22, 2019 the employer failed to ensure that effective machine guarding was in place on the small slitter line of the facility. No guarding was in place to protect employees from the hazard areas including but not limited to the slitter rolls, hold downs, and other moving machine parts. Barrier guarding and other means were not present.

Chemicals in the workplace include but are not limited to oils, solvents, and lubricants.
