Warehouse yard area; on or about 21 May 2019 and times prior thereto; the employer did not ensure that propane fuel cylinders were stored in a manner to prevent damage. Three instances were noted where stored cylinders were not secured:       a. Outside of paint locker area, ten propane cylinders of multiple quantities were not secured in a manner to prevent falling. Cylinders were stored free standing.       b. Recreation shop conex number two, two 4BW240 propane cylinders were not secured in a manner to prevent falling. Cylinders were stored free standing.       c. Recreation shop conex number one; one 4BW240 propane cylinder was not secured in a manner to prevent falling. Cylinder was stored free standing.  This condition exposed employees to contact with flammable and combustible liquids and other fire hazards.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards.  Garage bay adjacent to welding area; on or about 21 May 2019 and times prior thereto; the employer did not ensure that an oxygen cylinder used for welding was stored in a manner to prevent being knocked over or damaged. Administrative Building; on or about 21 May 2019 and times prior thereto; employees were not trained on the new Hazard Communications Standard (HCS 2012) provisions implemented on 1 December 2013. This training includes revised labeling elements and the new format for Safety Data Sheets. This condition exposed employees to inadequate training and chemical hazards.
