29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Jam Tire, Inc., on or about May 20th, 2019, at the location of 6202 Fairfield Dr., Northwood, OH, 43619: The employer did not ensure that employees performing work task such as, but not limited to, service of a water pump on a semi-tractor were protected from the hazards associated with unexpected start up of the engine through developed, documented, and utilized procedures. Employees operating in this condition were exposed to potential caught in, struck by, and crushing hazards.
