29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Worksite located at 2300 Independence Ave., Kansas City, Missouri- Employees were exposed to a fall hazard in that the ladder being used to access the 5th section scaffold was not extended at least 3 feet above the upper landing surface.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: Worksite located at 2300 Independence Ave., Kansas City, Missouri- Employees were exposed to fall hazards while working on a fabricated framed scaffold in which the scaffolding which was missing multiple guardrails on multiple sections of the scaffold.
