OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards when a strongback welding jig fell off the Fit-up #1 Rotator when an employee pulled on the strongback while troubleshooting the Rotator. Abatement documentation is required for this violation.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: Hardware, such as angle iron and pins, had not been provided by May 22, 2019 (injury date), for Fit-up #1 Rotator in order to de-energize the effect of gravity during rotation of the machine frame, that would prevent the strongback from falling.
