29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    a) On or about May 23, 2019 and at times prior thereto; A 10 foot front end attachment and a barrel attachment were used with their Hyster and CAT PITs to assist with loading cement trucks, but the fork attachment and barrel attachment had not been evaluated by an engineer or use approved by the manufacturer.        Note* Abatement certification IS required for this item.
