29 CFR 1926.403(b)(1)(iii): The employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees; sections of the electrical insulation on cords and cables was damaged. Shack on the north end of the jobsite. A miter saw with a frayed cord was used to cut wooden braces for the cement forms, exposing employees to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: Shack on the north of the project: The receptacle on a yellow extension cord powering a miter saw used for cutting wood braces was pulled from the cord exposing the internal wires. Employees were exposed to electrical and fire hazards.
