29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. 1) Self-Storage: On or about May 24, 2019, the employer exposed employees to fall hazards of approximately 15 feet, while working inside a scissor lift. An employee was standing and sitting on the guardrails of the scissor lift to elevate their height to access the roof in order to nail the metal roof down.

29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: 1) Self-Storage building: On or about May 24, 2019, the employer exposed employees to fall hazards approximately 21 feet high, in that all employees were not tied off while installing metal panels on the roof. 2) Self-Storage building: On or about May 24, 2019, the employer exposed an employee to fall hazards approximately 21 feet high, in that an employee was not wearing a personal fall arrest system with the lanyard attached to the building while conducting leading edge work nailing metal panels.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. 1) Self-storage: On or about May 24, 2019, the employer exposed employees to a fall hazard in that the 24 foot Werner extension ladder had a rung that was damaged. Employees were using the extension ladder to access and egress to the roof.
