29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:    a.  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1a for hazards associated with lack of energy control procedures and employee training.   b.  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1b for hazards associated with the employer failing to ensure that machines were lock out prior to being worked on.  c. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 2 for hazards associated with lack of powered industrial training.   d. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 3 for hazards associated with lack of guarding on the Cincinnati Press, Vipros 357 Press Brake, and Trumpf Press Brake.

29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative:  On or about May 20, 2019, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections to ensure employees performing servicing and maintenance activities on the Cincinnati Press Brake, Amada Vipros 357 punch press, and Trumpf press brake at the facility were protected from unexpected energizing, startup or release of store energy. Machines and equipment used throughout the facility are powered by electric, pneumatic, and hydraulic power.

29 CFR 1910.147(c)(2)(ii): If an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section shall utilize a lock out device, unless the employer can demonstrate that the tag out system will provide full employee protection as set forth in paragraph (c)(3) of this section:  On or about May 20, 2019, the employer failed to ensure that machines, such as but not limited to the Amada Vipros 357 punch press, were locked out prior to work being done on them such as but not limited to die change outs.

Powered industrial vehicles included but not limited to, the Nissan (Model #CYM02L255), Yale (Model #ERc100HBN48SE090SR), and Hyster (SN# B114V04140C).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.) On or about May 20, 2019, the employer failed to provide guarding on the back end of the Cincinnati press brake. Employees are exposed to struck by and caught in between hazards.  b.) On or about May 20, 2019, the employer failed to provide guarding in the front of the Amada Vipros 357 press brake. Employees are exposed to struck by and caught in between hazards.  c.) On or about May 20, 2019, the employer provided inadequate light curtain guarding on the Trumpf press brake.  Employees are exposed to struck by and caught in between hazards.
