29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) Built-Rite Construction, LLC at 956 Steele St., Denver, CO 802016: On or about 5/24/2019 the employer did not ensure that each employee on a scaffold was protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Two employees were standing on plywood placed over the fork attachments of a rough terrain fork lift approximately 20 ft. from the ground without fall protection. This condition exposed employees to fall hazards in excess of 6 ft.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, a safety net system, or a personal fall arrest system: a) Built-Rite Construction, LLC at 956 Steele St., Denver, CO 802016: On or about 5/24/2019, the employer did not ensure that each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, a safety net system, or a personal fall arrest system. Two employees were standing on the roof of a two story residential home approximately 20 ft. from the ground without fall protection. This condition exposed employees to fall hazards in excess of 6 ft.
