29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    (a)     Heritage Assisted Living- Kitchen:  On or about May 21, 2019 and at times prior: An electrical junction box which powered a Duke Manufacturing industrial oven had a knockout missing.    (b)     Heritage Assisted Living- Utility Basement:  On or about May 21, 2019 and at times prior: The left side breaker box had open spaces in #2 and #4 due to blank which did not cover the entire open space.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located within 8 feet of the floor had live parts exposed to employee contact:  (a)  Heritage Assisted Living- Memory Care Laundry Room: On or about May 21, 2019 and at times prior:  Unguarded/ Uncovered glass florescent light bulbs were located 7 feet 10 inches from the floor.    (b)  Heritage Assisted Living- Utility Basement: On or about May 21, 2019 and at times prior:  Unguarded/ Uncovered glass florescent light bulbs were located approximately 74.5 inches from the floor over the work bench, 81 inches from the floor over the sink area, and 79.5 inches from the floor throughout the rest of the area.   Glass florescent light bulbs were not guarded or covered to prevent accidental breakage or contact. This exposes employees to possible broken glass and electrical hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  Heritage Assisted Living-Resident Assistant Break Room:  On or about May 21, 2019 and at times prior:  A Magic Chef refrigerator and Stainless steel refrigerator were plugged into a surge protector (relocatable power tap).  The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a)  Heritage Assisted Living- Kitchen, Dishwashing Area:  On or about May 21, 2019 and at times prior:  An Ecolab spray bottle was not labeled with its contents- Ecolab peroxide multi surface cleaner- or hazard label warnings- corrosive, irritant. This exposes employees to possible respiratory irritation and/or dermatitis hazards.
