29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: a) On or about 5/28/19, at the site of Williamsville, NY. Employees, working on piping trenches and pits, were not protected from trip and fall hazard through the holes. The holes were up to 100 feet long, 18-24 inches wide, and 3-5 inches deep. NO ABATEMENT CERTIFICATION REQUIRED
