29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  (a) On the scaffold system located at the worksite located at 20200 Poplar Creek Parkway, Brookfield, WI, the employer did not ensure all sections where employees were working had full planking, thereby exposing workers to fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) At the worksite located at 20200 Poplar Creek Parkway, Brookfield, WI, the employer did not ensure a scaffold had a complete guardrail system near access points. There was a ladder installed within the scaffold system but the area around the access points did not have adequate fall protection.

29 CFR 1926.451(h)(1): 29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  (a) At the worksite located at 20200 Polar Creek Parkway, Brookfield, WI, the employer did not ensure employees were using head protection in addition other methods such as toe boards, screens, etc. to protect employees working below material, equipment and personnel, thereby exposing workers to struck-by hazards.    ...in the alternative...  29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) At the worksite located at 20200 Polar Creek Parkway, Brookfield, WI, the employer did not ensure employees were using head protection when conducting work from a scaffold system, thereby exposing workers to struck-by hazards.
