29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a.) Bil-Jax, Inc. - Archbold, Ohio: On or about May 23, 2019, the employer did not ensure employees were protected from amputation hazards while employees operated the tube bending machine. On May 23, 2019, an employee sustained lacerations and broken fingers injury that led to a partial finger amputation while manually reaching a metal tube placed in the bending machine that was not guarded, after unexpectedly activating the start button.
