29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: Clubhouse Roof: On or about May 29, 2019, employees were exposed to a fall hazard greater than 6 feet to the ground below while installing metal roof panels without fall protection.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: South side of clubhouse. On or about May 29, 2019, employees were exposed to a fall hazard while accessing the roof without the Werner ladder side rails extending at least three feet above the landing surface.
