29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer: On or about May 29, 2019, at the site, the employer failed to ensure the trench box was placed properly in accordance with the Xterra Trench shield tabulated data. The trench box was below the soil approximately 18 inches where employees were installing PVC pipe. Employees exposed to the hazard of being struck-by falling soil/rocks.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about May 28, 2019, at the site, the employer failed to ensure the employees were protected from cave-ins by an adequate protective system. Employees were installing communication lines outside the trench box which ranged in depth from approximately 6 feet to 8 feet. Employees exposed to cave-in hazards.
