OSH ACT of 1970 Section (5)(a)(1)  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees 29 CFR 1910.23(c)(4): Ladders were not used on stable and level surfaces and were not secured or stabilized to prevent accidental displacement:   �On or about May 28, 2019, during the relocation of a grain bin, an employee worked from an unstable extension ladder positioned on a moving vehicle and positioned himself on top of the grain bin and was not protected from a fall of approximately 23.8 feet.  �Among other feasible and acceptable methods of abating the hazards and eliminating the need to access the ladder or top of grain bin are: (1) relocating the grain bin by using a millwright to disassemble the grain bin at one location and reassemble the grain bin at a second location; or (2) contracting with the local electric utility to relocate any electrical wires impeding passage of the grain bin on a public road.�    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started:  a)    Planned route from 6149 S Route 64, Clare, IL to 27668 Esmond Rd., Esmond, IL. On or about May 22, 2019, employees were exposed to electrical shock and burn hazards from energized power lines while measuring power line heights with a non-rated fiberglass pole.  Employees stood on the back of a truck and used the pole to take the measurements, touching the lines with the pole at times.   The lines were not deenergized and no electrical protection was utilized by the employees engaged in this task.    b)    Transport route from 6149 S Route 64, Clare, IL to 3646 Old State Road, Kirkland, IL.  On or about May 28, 2019,  employees were exposed to electrical shock and burn hazards when manually raising energized overhead power lines with a non-rated fiberglass pole.  An employee stood on a ladder on the back of a trailer hauling a grain bin to raise the power lines with the pole to allow passage beneath.  No electrical protection was utilized by the employees engaged in this task.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Route 64, Clare, IL to 3646 Old State Road, Kirkland, IL.  On or about May 28, 2019, the employer did not ensure that a semi-trailer that was hauling a grain bin maintained at least 4 feet of clearance from energized overhead power lines.  This exposed employees to electrical shock, burn and struck-by hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained:  3646 Old State Road, Kirkland, IL.  On or about May 28, 2019, the employer did not ensure that a Snorkel TB80 aerial work platform was operated so it maintained 10 feet of clearance from overhead power lines.  An employee attempted to use the aerial platform to raise energized overhead power lines so a semi-trailer that was hauling a grain bin could pass beneath.  This exposed employees to electric shock, burn and struck-by hazards.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
