29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    Location: North side of residential building Rodman St. side, Fall River, MA. 02720    On or about 5/29/19 employees were exposed to head injuries while working on the ground 40 feet below the employees working on the roof above them. Employees on the roof were dropping debris to the ground below.  Head protection was not used or provided to the employees on the ground.

29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):    Location: North side of residential home, Rodman St. side, Fall River, MA 02720    On or about 5/29/19 employees working on the ground were exposed to debris being dropped from the employees working on the roof 40 feet above.  A chute or an equivalent means was not provided or used at the time of the inspection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Location: 219 Rodman St. Fall River, MA 02720 existing residential home.    On or about 5/29/19 employees were exposed to fall hazards of 40 feet to the ground below where a guardrail system or personal fall arrest system was not being used or properly used at the time of the inspection.
