29 CFR 1926.350(a)(10): Oxygen cylinder in storage was not separated from fuel gas cylinders or combustible materials a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire resistance rating of at least one-half hour:    a)  331 June Avenue, Blandon PA - An oxygen tank was observed stored directly next to an acetylene tank, on or about May 29, 2019.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)  331 June Avenue, Blandon PA - An employee secured a tarp on the roof of the building and was not protected from an approximate 24 foot fall to the ground below, on or about May 29, 2019.  No abatement certification or documentation required.

29 CFR 1926.760(a)(1):  Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:  a)  331 June Avenue, Blandon PA - Employees cut steel beams to free a 30 foot tall steel frame tower from the roof of a building and were not protected from an approximate 24 foot fall to the ground below, on or about May 21, 2019.  No abatement certification or documentation required.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a)  331 June Avenue, Blandon PA - On or about May 29, 2019, an employee was not protected from an approximate 24 foot fall to the floor below while securing a tarp to cover a roof opening created after a steel tower was removed from it.  There were sections of metal decking placed over the roof opening, however they had not been tightly laid.     No abatement certification or documentation required.
