29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. (a) Worksite On or about 5/30/2019, the employer did not ensure that flexible cords were inspected, and those cords that were missing ground pins were taken out of service prior to use.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: (a) Worksite On or about 5/30/2019, employees were not protected from electric shock and burns while using a flexible electrical cord that was pulled out from its plug end, which exposed the inner wires from the cord outer casing.
