29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a). On or about 05/24/2019, The employer required the employees to don a 3M N95 8210 and 3M 1/2 mask respirators while cutting, polishing, and grinding marble and stone products. The employer did not implement a respiratory protection program to include a written respiratory program, medical evaluations, fit tests, and training.   Putnam Granite was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(1) which was contained in OSHA inspection number 1404308, citation number 1, item number 1a and was affirmed as a final order on 4/16/19, with respect to a workplace located at 100 Buckshollow Rd, Mahopac, NY 10541.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a). On or about 05/24/2019, The employer has the employees use AKEMI Platinum Adhesive and AKEMI Hardener to fill in small imperfections on the stone.  AKEMI Platinum Adhesive contains chemicals such as, but not limited to, Styrene.  AKEMI Hardener contains chemicals such as, but not limited to dibenzoyl peroxide. Employees use this product on 3-4 times a day for a couple of minutes. The employer did not implement a hazard communication program to include a written program, training on the hazards associated to using the chemicals and SDS's.   Putnam Granite was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(e)(1) which was contained in OSHA inspection number 1404308, citation number 1, item number 2a and was affirmed as a final order on 4/16/19, with respect to a workplace located at 100 Buckshollow Rd, Mahopac, NY 10541.
