29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) Recreational vehicles: On or about May 30, 2019 and times prior thereto the employer exposed employees to fall hazards of up to 10 feet, 11 inches to the ground below when walking-working on air conditioning units and vents on top of recreational vehicles. The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems.  b) Mezzanine floor: On or about May 30, 2019 and times prior the employer exposed employees to falls hazards of  up to 8 feet, 8 inches to the lower level when walking-working on mezzanine floor with unprotected side . The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems.
