29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: (a) Derek, Inc. on or about May 30th, 2019, at the location of 35613 Chester Rd., Avon, OH 44011: A synthetic sling with visible snags and tears was not inspected in order to identify these defects and protect the employee. The employee was exposed to crushing hazards while working in the immediate proximity of a load suspended by the damaged sling.
