29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: On, or about, June 11, 2019, at 158 West Main Road, Conneaut, OH 44030, the employer failed to maintain records of workplace injuries and illnesses on the required OSHA Form 300, or equivalent for the calendar year 2019 when an employee sustained a privacy type injury in March of 2019 that met the general recording criteria, but was not included on the OSHA 300 log for the facility.
