DBA 1st Choice Roofing Company on or about May 30, 2019, at the address of 301 Broadway, Lorain, OH: The employer failed to ensure the supported scaffolding level from which employees were performing roofing work was fully planked. The employees were exposed to a 12 ft. fall to the ground below.

The employee was exposed to a 12ft fall to the level below while feeding roofing matrial to another employee on the roof deck without the benefit of fall protection, guardrails or a fully decked platform.
