29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) On or about 05/25/2019, at a jobsite located at 4398 N Corrington Ave, KC MO, an employee was cutting plywood with his skill saw. The skill saw hit a hard knot, causing the saw to jump and resulted in his ring and middle fingers of his left had to be amputated. The employer did not report the amputation to OSHA until 05/28/2019, more than 24 hours after the injury.
