29 CFR 1926.502(i)(3): All covers shall be secured when installed so as to prevent accidental displacement by the wind, equipment, or employees.  a) North Side 1st Floor: Employees engaged in stripping formwork were exposed to fall hazards of approximately 10 feet while working around floor openings that the covers were not secured to prevent accidental displacement, on or about 5/31/2019. On 5/31/2019, an employee fell through a floor opening where the plywood cover was removed by another employee to run a pump hose through.
