29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  a) Essentra Plastics, LLC DBA Essentra Components, 2614 McClelland Avenue, Erie, PA -  The company did not provide annual audiometric testing for employees exposed to noise above the action level.  On or about June 13, 2019, the following noise exposures were measured:  1. A machine operator was exposed to a mixed noise level of 95.4 dBA or an equivalent dose of 212.5%, as an 8-hour TWA, while operating dip mold machine #3. This level was based on 445 minutes of sampling; zero exposure was considered for the unsampled time. Audiometric testing was last provided May 2018; and    2. A machine operator was exposed to a mixed noise level of 88.6 dBA or an equivalent dose of 82.78%, as an 8-hour TWA, while operating dip mold machine #4. This level was based on 471 minutes of sampling; zero exposure was considered for the unsampled time. Audiometric testing was last provided April 2018.

29 CFR 1910.95(g)(7)(i): Each employee's annual audiogram was not compared to that employee's baseline audiogram to determine if the audiogram is valid and if a standard threshold shift as defined in 29 CFR 1910.95(g)(10) has occurred:  a) Essentra Plastics, LLC DBA Essentra Components, 2614 McClelland Avenue, Erie, PA -  The company did not ensure employee annual audiograms were compared to that employee's baseline audiogram to determine if the audiogram was valid and if a standard threshold shift had occurred:  On or about June 13, 2019, the following noise exposures were measured:  1. A machine operator was exposed to a mixed noise level of 95.4 dBA or an equivalent dose of 212.5%, as an 8-hour TWA, while operating dip mold machine #3. This level was based on 445 minutes of sampling; zero exposure was considered for the unsampled time; and    2. A machine operator was exposed to a mixed noise level of 88.6 dBA or an equivalent dose of 82.78%, as an 8-hour TWA, while operating dip mold machine #4. This level was based on 471 minutes of sampling; zero exposure was considered for the unsampled time. Audiometric testing was last provided April 2018.   Baseline audiograms were not provided to the audiometric testing company for comparison to 2018 audiometric tests.

On or about June 13, 2019, the following noise exposures were measured:  1.
