OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. On or about May 2, 2019, employees operating a Split-Fire, Model 4290 log splitter machine were exposed to caught-in and point of operation machine hazards while loading and operating the log splitter machine that was not designed by the manufacturer to be operated with two operators and/or equipped with controls that required simultaneous operation from each operator.
