Warehouse: The employer did not ensure through visual observation and/or specific instruction that the passageway and/or working-walking surface was kept clean from material, such as (but not limited to) acrylic.    On or about 07/10/2019, an employee slipped on a piece of acrylic and sustained a broken foot injury.
