29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a) Building Exterior: Employee exposed to a 12 foot fall hazard while working from a fabricated scaffold two frames in height that was not equipped with a guardrail system or other means of fall protection in place, on or about 5/31/2019.  b) Building Exterior: Employee exposed to a fall hazard while using a wooden pallet on the forks of a Lull telescopic Powered industrial truck approximately 16 feet above the ground, on or about 5/31/2019.

EACH 1910 STANDARD USED MUST INCLUDE "AND AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d)" at the end of the SAVE.   a) Building Exterior: The employer did not ensure that the operator of a Lull Telescopic Powered industrial truck had successfully completed training on the safe operation of a Lull Telescopic Powered industrial truck, on or about 5/31/2019.
