29 CFR 1910.22(a)(2): (2) The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided. At the establishment; the employer did not ensure all work areas and walkways were maintained in a clean and, to the extent feasible, in a dry condition: a) the roof leaks and water is on the work floors causing slip and fall hazards, and b) the passage way used to retrieve merchandise was obstructed with buckets of standing water from the leaking roof causing slip, trip and fall hazards. c) old merchandise is stored in the passage ways and work area causing trip hazards.

The white cord was spliced to a yellow extension cord and wrapped with black tape, exposing employees to the hazard of electrical burns and shock.
