Employee's were exposed to struck by hazards from forklifts. Forklift operators were not trained with training consisting of formal instruction, and practical training and evaluation. There was no written and practical evaluation of the forklift operator's driving.

There were two sumps with augers in the grain bin # 8 which were not guarded. On or about May 27, 2019, an employee received an amputation injury when emptying the grain in bin # 8.
