Worksite: The employer did not ensure that an inspection of the excavations/trench, adjacent area and protective system was made by a competent person for evidence of a situation that could have resulted in a cave-in, a failure of the protective system or other hazardous conditions. Katchko & Sons Construction, Inc. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.651(k)(1) which was contained in OSHA Inspection Number 1172377, Citation Number 1, Item Number 2b, issued on 10/27/2016 with respect to a workplace located at 58 Doolittle Road, Stamford, CT and became a final order on 11/29/2016.
