29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:    a) Residential Home, 507 E 6th Street, Helena, MT: On or about May 21, 2019 employees were exposed from a fall height of approximately 16 feet while working on a roof with a 10:12 pitch to the concrete below; without the use of fall protection.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder:  a) Residential Home, 507 E 6th Street, Helena, MT: On or about May 21, 2019 employees were exposed from a fall height of approximately 16 feet while working on a roof with a 10:12 pitch to the concrete below; the Werner extension ladder was not extended three feet above the lower roof line, where the employees were egressing the ladder.
